New information has emerged following the death of Jaqueline “Jackie” Falk, the youngest daughter of legendary Columbo actor Peter Falk. An autopsy report has provided additional details about the circumstances surrounding her passing earlier this year.

Jaqueline Falk, 60, was discovered at her home in Los Angeles on April 27 after a concerned friend requested a welfare check when repeated attempts to contact her went unanswered. Investigators believe she had died several days earlier, with authorities estimating her death occurred around April 22.

The autopsy confirmed that Jaqueline died by suicide. Multiple handwritten notes were recovered from the garage, where she was found. Officials also determined there was no evidence of alcohol or drugs in her system at the time of her death.

Jaqueline was one of two daughters adopted by Peter Falk and his first wife, Alyce Mayo, during their marriage. Her death has deeply saddened admirers of the acclaimed actor, whose portrayal of the sharp-minded detective Lieutenant Columbo made him one of television’s most recognizable stars.

Peter Falk rose to international fame after Columbo debuted in 1971. Throughout the series’ successful run, he earned four Primetime Emmy Awards and a Golden Globe, cementing his place among television’s most celebrated performers.

Jaqueline’s sister, Catherine Falk, has long spoken about the close bond within their family. She has recalled that despite Peter Falk and Alyce Mayo divorcing in 1976, they remained on friendly terms and continued to share a respectful relationship while raising their daughters.

Following Peter Falk’s diagnosis with advanced dementia in 2008, Catherine became an outspoken advocate for families facing visitation and guardianship disputes. After the actor’s death in 2011, she established the Catherine Falk Organization to promote legal reforms aimed at protecting the rights of relatives caring for elderly family members.

The organization continues to campaign for expanded family visitation rights, elder abuse prevention measures, and guardianship reform. Its efforts have also supported legislation commonly known as the Peter Falk Law, which has been considered by lawmakers in numerous U.S. states.

Jaqueline was preceded in death by both of her parents. Peter Falk died in 2011 at the age of 83, while Alyce Mayo passed away five years later at age 85. Catherine Falk is now the remaining member of the immediate Falk family.
